All applicants shall be pregnant, breastfeeding, or postpartum women, infants, or children under age 5 as verified by information such as one of the following:

1. (a) Health care provider documentation of applicant’s pregnancy unless visibly pregnant; or
2. (b) Applicant’s birth certificate containing date of birth as documentation of age.
