Procedures for Patients to Reverse Decision to Opt-in or Opt Out of the Registry

- (a) A patient, or the patient’s parent or guardian if the patient is a minor, may reverse the decision to opt-in or opt-out of participation in the registry at any time by informing each of their administering health care providers who participate in the registry of their decision to reverse their opt-in or opt-out status.
- (b) The provider shall document the patient’s, or the patient’s parent or guardian if the patient is a minor, reversal of decision and explicit consent to opt-in, if applicable, in the patient’s medical record.
- (c) The provider shall ensure that past, current, and future immunization information is or is not further reported to the registry in accordance with the patient’s change in opt-in or opt-out status.
- (d) If a patient, or the patient’s parent or guardian if the patient is a minor, reverses their decision and opts-out of the registry, they may withdraw their information from the registry, pursuant to He-P 307.07.
