An applicant shall be eligible for admission to Glencliff home if that applicant:

1. (a) Is 60 years of age or older, or younger if the administrator determines that the applicant:

   1. (1) Has been referred to Glencliff home by an area agency, a community mental health program, a licensed practitioner, or New Hampshire hospital;
   2. (2) Meets all of the other criteria in (b)-(f) below; and
   3. (3) Has been rejected by at least 2 other facilities;
2. (b) Has a mental illness or developmental disability;
3. (c) Does not require physician services or psychiatric care on a daily basis;
4. (d) Has qualified for nursing facility level of care in accordance with criteria and procedures set forth in He-E 802.04, if the applicant is a medical assistance recipient;
5. (e) Voluntarily requests admission or has a guardian of the person appointed pursuant to RSA 464-A who requests admission and has obtained the approval of a probate court for such admission in accordance with RSA 464-A:25; and
6. (f) Has been reviewed and approved for nursing facility placement pursuant to He-M 1302.
