1. (a) In order to receive Medicaid reimbursement for community participation services, area agencies, as the enrolled providers of home and community‑based care services, shall submit claims for payment to:

2. (b) Payment for Medicaid waiver services shall only be made if prior authorization has been obtained from the bureau pursuant to He-M 517.08.

3. (c) Requests for prior authorization shall be made in writing to:

   105 Pleasant Street

   Concord, NH 03301
