Policy and Procedure Requirements

Each provider agency shall establish and implement written policies which shall specifically address the following:
(a) Non-discrimination on the basis of:
- (1) Race;
- (2) Color;
- (3) Sex;
- (4) Creed;
- (5) National origin;
- (6) Age;
- (7) Marital status;
- (8) Familial status;
- (9) Sexual orientation; or
- (10) Physical or mental disability; and
- (b) Knowledge, skills, and abilities relative to providing HCBS waiver services.
