1. (a) All suspensions and terminations shall comply with the procedures identified in He-M 401.

2. (b) Whenever an individual has been suspended or terminated from a program or service, a note shall be entered into the clinical record and include:

   1. (1) If a suspension of services, the effective date and length of the suspension of services;
   2. (2) If a termination of services, the effective date of the termination of services;
   3. (3) The reason(s) for the suspension or termination;
   4. (4) The approval of the appropriate CMHP personnel; and
   5. (5) Documentation that notification of the suspension or termination has been provided to the individual.
