1. (a) Once the individual has been determined eligible to receive Title III services, the following Title III services shall be authorized by the contract agency, acting on behalf of BAAS, in order for the individual to receive services funded by Title III:

   1. (1) Adult day program services;
   2. (2) Adult in-home care services;
   3. (3) Home health aide services;
   4. (4) Homemaker services;
   5. (5) Nutrition services: congregate meals; and
   6. (6) Nutrition services: home delivered meals.
2. (b) Service authorizations shall consist of the specific types of services required to meet the needs identified on the individual’s person-centered plan.
