1. (a) Pursuant to RSA 135-E:3, III, the county attorney, the attorney general, or the agency with jurisdiction may request that the MDT assess and evaluate a person to determine if the person meets the definition of sexually violent predator.

2. (b) The request to the MDT in (a) above shall:

   1. (1) Be made in writing; and

   2. (2) Include the following information about the person:

      1. a. Personal information shall include:

         1. 1. Full name;
         2. 2. Current age;
         3. 3. Date of birth; and
         4. 4. Unique personal identification number assigned by the agency with jurisdiction;

      2. b. Information about the offense(s) giving rise to the request in (a) above shall include:

         1. 1. RSA reference and crime descriptor;
         2. 2. Copy of the indictment(s);
         3. 3. Disposition of charges;
         4. 4. Sentence(s), where applicable;
         5. 5. Prosecuting entity; and
         6. 6. Court of conviction; and

      3. c. Total confinement information shall include:

         1. 1. Place of total confinement;
         2. 2. Date(s) of incarceration or admission to the secure psychiatric unit or the New Hampshire hospital, as applicable;
         3. 3. Minimum parole date and maximum date of release, if applicable; and
         4. 4. Anticipated date of release from total confinement, if applicable.
3. (c) The request to the MDT in (a) above may include any other information relevant to a determination of whether the person meets the definition of sexually violent predator, including, but not limited to, the information and documents described in He-C 701.05(a).

4. (d) The request to the MDT in (a) above, and any other information pursuant to (c) above, shall be delivered to the department at the following address:
