1. (a) A request for certification reconsideration shall:

   1. (1) Be filed within 30 days of the date of receipt of the letter sent by DCYF described in He-C 6340.17;
   2. (2) Be submitted in writing; and
   3. (3) Be filed with the director of DCYF.
2. (b) The DCYF director shall uphold or overturn the request pursuant to He-C 6340.18.
3. (c) The applicant or provider shall be notified, in writing, of the decision by the director.
4. (d) The applicant or provider may appeal the DCYF director’s decision pursuant to He-C 6340.19.
