1. (a) If any child while in the care of the program sustains a serious injury, as defined in He-C 4002.01(bk), or otherwise requires emergency services as prescribed in first aid and CPR training, the program manager or designee shall:

   1. (1) Immediately notify emergency personnel and the child’s parents or guardians; and
   2. (2) Notify the department within 48 hours via NHCIS.

2. (b) The program manager or designee shall provide a written report by the next business day to the parents or guardians of the child or children that sustained a serious injury to fully inform them of the details of the incident reported in (a) above, including, if known:

   1. (1) The name of individuals involved in, and who witnessed, the incident, while keeping the identities of other children confidential;
   2. (2) What occurred prior to and following the incident;
   3. (3) When and where the incident occurred; and
   4. (4) Any action taken, or that will be taken by the program in response to the incident.

3. (c) The program manager or designee shall immediately notify a child’s parent or guardian if their child sustains a bump or injury to their head or face that is more than a minor injury such as a scrape or scratch, resulting in any one of the following:

   1. (1) Excessive bruising or swelling;
   2. (2) An increase in fussiness or sleepiness;
   3. (3) Dizziness, clumsiness, or trouble with coordination;
   4. (4) Nausea or vomiting;
   5. (5) Loss of consciousness;
   6. (6) Headache;
   7. (7) Speech, vision, or hearing impairment; or
   8. (8) Discharge or blood from the bump.
4. (d) Except as required in (b) above, when a child sustains an injury pursuant to (c) above, the program manager or designee shall provide written notification to the child’s parent or guardian as soon as possible on the date of the injury, detailing how the injury occurred, the date, time, and location of the injury, a description of the first aid treatment given to the child, and the name of the staff who administered first aid.
5. (e) First aid beyond cleaning a minor scrape or applying a cold cloth or an adhesive bandage, shall only be administered by staff currently certified in first aid.
