1. (a) A department representative shall complete a monitoring report within 14 business days of an on-site visit, which shall include:

   1. (1) Specific recommendations for provider improvements and changes, if any;
   2. (2) Corrective actions to be taken by the provider, if any; and
   3. (3) Time frames for any corrective actions indicated.
2. (b) The department representative shall forward a copy of the monitoring report to the program director.
