Each audiologist whose license has been expired for not more than 2 years shall, pursuant to RSA 137-F:13, III, have his or her license restored upon submitting:

1. (a) A completed renewal application as required by Hcp 401.03;
2. (b) Payment of the restoration fee described in Hcp 405; and
3. (c) Documentation that the applicant has met the continuing education requirements of Hcp 402.
