1. (a) The licensee shall receive continuing professional education credit for the performance of formal mentored independent study in genetic counseling so long as the study is performed under a written and signed mentorship agreement setting forth:

   1. (1) The name of the mentor; and
   2. (2) The goals and objectives to be met by the mentored licensee during the course of the mentored licensee’s study.
2. (b) The licensee shall receive one hour of continuing professional education credit for every 2 clock hours of formal mentored independent study, to a maximum of 8 hours per renewal cycle.

3. (c) Documentation of formal mentored independent study in genetic counseling shall be:

   1. (1) A photocopy of the signed mentorship agreement; and

   2. (2) The licensee’s signed written statement describing the independent study showing:

      1. a. The dates of the study;
      2. b. The hours spent on the study; and
      3. c. The focus and outcome of the study.
