1. (a) The licensee shall receive continuing professional education credit for the successful completion of non-academic courses, seminars, conferences, and workshops on the following topics:

   1. (1) Genetic counseling theory or practice, or both; or
   2. (2) Any conditions for which genetic counseling is often or usually recommended.
2. (b) For each clock hour of attendance at such courses, seminars, conferences, and workshops, the licensee shall be credited with one hour of continuing professional education credit.

3. (c) Documentation of successful completion of non-academic courses, seminars, conferences, and workshops, including grand rounds, shall be a photocopy of an official certificate showing:

   1. (1) The licensee’s name;
   2. (2) The name and location of the course, seminar, conference, workshop, or grand rounds;
   3. (3) The name of the provider or sponsoring entity;
   4. (4) The beginning and ending date(s) of the program, course, seminar, conference, workshop, or grand rounds;
   5. (5) The number of hours credited by the provider or sponsoring entity or the brochure showing the scheduled hours; and
   6. (6) The signature or stamp of the speaker, the instructor, or a representative of the provider or sponsoring entity.
