Documents Required of Applicants For Provisional Licensure

Applicants for provisional licensure shall provide, or arrange for the OPLC to receive, the following documents supporting their applications:

(a) Documentation that the applicant has been authorized to take a certification exam by a certifying agency; and
(b) The supervision form described in Gen 306.06.
