1. (a) Upon employment of an unlicensed individual the funeral home owner/manager shall register the employee with the board, in writing, within 30 days.

2. (b) Registration shall include:

   1. (1) The individual’s name;
   2. (2) Address;
   3. (3) Date of employment and
   4. (4) Employment status, either full-time at 40 hours per week or part-time at less than 40 hours per week or per-diem.
