1. (a) Each apprentice embalmer shall assist in embalming 25 bodies according to (e) below. Assisted embalmings shall be completed prior to unassisted embalmings.
2. (b) Each apprentice embalmer shall embalm 25 bodies unassisted according to (e) below.
3. (c) Each apprentice embalmer shall arrange the details required for conducting 25 funerals along with conducting the arrangement conference, assisting families with selection of merchandise, and other phases of the profession.

4. (d) Each apprentice embalmer shall complete 25 “Apprentice Funeral Directing Form” revised 9/2025 by providing the following:

   1. (1) Consecutive apprentice funeral directing form number;
   2. (2) Name and license number of apprentice embalmer;
   3. (3) Name and license number of sponsor;
   4. (4) Name and address of funeral home sponsoring apprentice;
   5. (5) Name of deceased;
   6. (6) Place and date of death of deceased;

   7. (7) Type of funeral service, checking all that apply:

      1. a. Full service funeral;
      2. b. Calling hours;
      3. c. Memorial service;
      4. d. Graveside service; or
      5. e. Direct cremation;
   8. (8) Place and time of funeral service;
   9. (9) Name and denomination of officiating clergy;

   10. (10) List of funeral participation, checking all that apply;

       1. a. Assisted in filing death certificate;
       2. b. Assisted family with selection of merchandise;
       3. c. Assisted with arrangement conference;
       4. d. Arranged for clergy;
       5. e. Arranged flowers;
       6. f. Arranged for military honors;
       7. g. Parked cars;
       8. h. Ordered casket or vault;
       9. i. Directed traffic;
       10. j. Drove family car;
       11. k. Drove hearse;
       12. l. Prepared newspaper notices;
       13. m. Received visitors;
       14. n. Drove service car;
       15. o. Seated guests at service;
       16. p. Contacted cemetery;
       17. q. Arranged for cremation;
       18. r. Arranged for fraternal services;
       19. s. Assisted with Social Security and Veterans Administration forms;
       20. t. Assisted to obtain a disposition permit;
       21. u. Assisted with office work; or
       22. v. Carried casket;
   11. (11) Signature of apprentice embalmer and date; and
   12. (12) Signature of sponsor and date.

5. (e) Each apprentice embalmer shall complete an “Apprentice Embalming Form” revised 9/2025 for each embalming completed in (a) and (b) above, by providing the following:

   1. (1) Either the unassisted embalming report number, or the assisted embalming report number, using consecutive numbers in chronological order;
   2. (2) Name and license number of apprentice embalmer;
   3. (3) Name and license number of sponsor;
   4. (4) Name and address of funeral home sponsoring apprentice;
   5. (5) Name and date of death of deceased;
   6. (6) Place and time of death;
   7. (7) Primary cause of death;
   8. (8) Date and time body received at funeral home;
   9. (9) Name of funeral home receiving body;
   10. (10) Name of attendants on first call;

   11. (11) Pre-embalming condition of body, checking all that apply;

       1. a. Straight;
       2. b. Partial or full autopsy;
       3. c. Emaciated or dehydrated;
       4. d. Edematous or dropsical;
       5. e. Purge;
       6. f. Tissue gas;
       7. g. Odor;
       8. h. Skin slip;
       9. i. Discoloration;
       10. j. Rigor mortis;
       11. k. Mutilations;
       12. l. Jaundice;
       13. m. Organ or tissue donor, describe;
       14. n. Surgery;
       15. o. Exposure to temperature extremes or
       16. p. Other, describe;
   12. (12) Time between death and embalming;
   13. (13) Body refrigerated, indicating ‘yes’ or ‘no’;
   14. (14) Arteries used for injection;
   15. (15) Veins used for drainage;
   16. (16) Type and index of arterial fluid used, rate of flow;
   17. (17) Method of injection, hand pump, gravity, or machine pressure;
   18. (18) Concentration of arterial fluid injected, and number of gallons injected;
   19. (19) Type and amount of cavity fluid used;
   20. (20) Other supplemental chemicals or fluids used;

   21. (21) Occurrences during embalming, checking all that apply;

       1. a. Clearing;
       2. b. Purge;
       3. c. Distention of face or hands;
       4. d. Firming;
       5. e. Leakage; or
       6. f. Vascular problems;
   22. (22) How any problems were encountered and rectified, with a description;

   23. (23) Checking the following procedures completed;

       1. a. Undressed and washed body;
       2. b. Disinfected and packed orifices;

       3. c. Shaved;

          d Set features, including closing eyes and mouth;

       4. e. Inserted cannula drain tubes;
       5. f. Mixed and injected fluids;
       6. g. Aspirated cavities, injected cavity fluid;
       7. h. Treated cavities and viscera for autopsy;
       8. i. Reaspirated cavities;

       9. j. Sutered and sealed incisions;

          k Treated skin slip and ulcers;

       10. l. Washed body and prepared for dressing;
       11. m. Dressed and casketed body;
       12. n. Applied cosmetics;
       13. o. Applied restorative measures;
       14. p. Cleaned and sanitized table, floor, prep room;
       15. q. Washed and sterilized instruments;
       16. r. Hypodermic treatment; or
       17. s. Chose injection site, indicating incisions, raised vessels;
   24. (24) Embalming operation complications, indicating “yes” or “no”;
   25. (25) Description of embalming complications and methods used for correction;
   26. (26) Signature of apprentice embalmer and date; and
   27. (27) Signature of sponsor and date.
