Renewal Application

Each applicant for licensure renewal shall submit:
- (a) The “Universal Application for License Renewal” as required by Plc 308.05(a);
- (b) The “Addendum to the Universal Application for License Renewal” as required by Fors 401.05(b); and
- (c) The fee required by Plc 1002.16.
