1. (a) Permittees shall maintain a current record of all wildlife received or released which shall include the name and address of the permittee.

2. (b) For each animal received, the record shall include the following:

   1. (1) Species;
   2. (2) Date wildlife was found;
   3. (3) Location where wildlife was found;
   4. (4) Date of admission;
   5. (5) Name, address, and telephone number of the individual from whom acquired;
   6. (6) Reason for admission;
   7. (7) Date(s) of veterinary evaluation or treatment;
   8. (8) Disposition;
   9. (9) Date of disposition; and
   10. (10) Release site.
3. (c) Permittees shall maintain records for 5 years.
4. (d) Permittees shall submit to the department a copy of all records, kept pursuant to (b) above, each year by January 31 of the following calendar year.
5. (e) Wildlife rehabilitation facilities and records shall be available for inspection by fish and game department personnel at any time during ordinary business hours for the purpose of ascertaining compliance with this part.
6. (f) Department personnel shall remove wildlife from the custody of the permittee if it is discovered that the permittee is not in compliance with Fis 812.02(a), (b), (e), (f), (g), or (h) or that is determined to pose significant risk for RVS pursuant to Fis 812.01(i).
