1. (a) Any firm, corporation, or person engaged in the taking or landing of herring pursuant to Fis 603.07 or the processing of shrimp pursuant to Fis 607.01 shall obtain a permit from the executive director.

2. (b) The applicant shall provide:

   1. (1) The business owner’s:

      1. a. Name and maiden name, if applicable;
      2. b. Street and mailing address, if different;
      3. c. Height and weight;
      4. d. Hair and eye color;
      5. e. Home telephone number; and
      6. f. Signature of permittee subject to the penalties for unsworn false statements under RSA 641:3.

   2. (2) The businesses:

      1. a. Name of business;
      2. b. Federal Tax Identification number;
      3. c. Date of Incorporation;
      4. d. Business street address;
      5. e. Business telephone number;
      6. f. Type of business;
      7. g. Name of contact person;
      8. h. Business name of processing operations harvest sold to; and
      9. i. Processing operations street address.

   3. (3) The vessels:

      1. a. Vessel owner, if different than applicant;
      2. b. Vessel owner street address, if different than applicant;
      3. c. Vessel name;
      4. d. State of registration;
      5. e. State registration or coast guard number;
      6. f. National Marine Fisheries Service federal permit number;
      7. g. Principal port;
      8. h. Hull ID number;
      9. i. Hull construction material;
      10. j. Vessel length;
      11. k. Year built;
      12. l. Gross and net tons, if federally documented vessel;
      13. m. Horsepower, if federally documented vessel;
      14. n. Hold capacity in tons, if federally documented vessel; and
      15. o. Crew size.
