1. (a) Any person who possesses a lobster tail processors permit as specified in Fis 609.03 shall submit an annual report to the executive director by January 31 for the previous calendar year which includes:

   1. (1) Name and address of business, firm or corporation
   2. (2) Lobster tail processing permit number;
   3. (3) Wholesale dealers license number;
   4. (4) Number of lobster tails processed at the facility each month of the calendar year;
   5. (5) Name and address of dealers to whom the lobster tails were sold;
   6. (6) Reporting year; and
   7. (7) Signature of permittee subject to the penalties for unsworn false statements under RSA 641:3.
2. (b) Any permittee failing to report shall not be issued a permit until such time that the required information in (a) has been submitted.
