1. (a) All persons reporting a turkey kill to a turkey registration station under RSA 209:12-a shall provide the following:

   1. (1) The shooter’s:

      1. a. Name;
      2. b. Street and mailing address;
      3. c. Telephone number;
      4. d. Residency; and
      5. e. Date of birth;
   2. (2) Town of kill;
   3. (3) Date of kill;
   4. (4) Time of kill;

   5. (5) The following information about the turkey:

      1. a. Sex;
      2. b. Weight;
      3. c. Length of beard to the nearest ¼ inch;
      4. d. Length of left and right spurs;
      5. e. Age, by indicating whether the turkey is adult or immature; and
      6. f. Description of any abnormality, injury, off coloration, leg band, or wing streamer on turkey;
   6. (6) Weapon type used;
   7. (7) Wildlife management unit of kill;

   8. (8) The following license information:

      1. a. Hunting license type(s) and license number(s) held by licensee; and
      2. b. Permit type(s) and permit number(s) held by licensee and under which the turkey was taken; and
   9. (9) The registration number of the vehicle transporting the turkey.

2. (b) The registration agent shall include the following:

   1. (1) The date and time of turkey registration;
   2. (2) The date and time of turkey license purchase; and
   3. (3) The registration station number.
3. (c) The hunter shall sign the paper report subject to the penalties for making unsworn false statements under RSA 641:3.

4. (d) The registration agent shall:

   1. (1) Provide the hunter with a turkey confirmation number; and
   2. (2) Sign the paper report or file the electronic report subject to the penalties for making unsworn false statements under RSA 641:3.
