All persons reporting a bear kill to a New Hampshire conservation officer, fish and game personnel, or registration agent authorized by the director shall provide the following information on the “Bear Registration Form” subject to the penalties for making unsworn false statements under RSA 641:3:

1. (a) The hunter’s:

   1. (1) Hunting license type, whether resident, non-resident, or other;
   2. (2) Hunting license number;
   3. (3) Bear permit number, if different;
   4. (4) Indication to having a permit to use dogs to take bear;
   5. (5) Indication to having viewed the bear hunting education materials on the department website;
   6. (6) Name;
   7. (7) Mailing and street address;
   8. (8) Date of birth; and
   9. (9) Telephone number;
2. (b) The wildlife management unit and town where the bear was killed and specific locality within the town;
3. (c) The cause of death;
4. (d) The date and time of kill;
5. (e) Whether or not a pre-molar was collected;
6. (f) The dressed weight and sex of the bear;
7. (g) If female, lactation status;
8. (h) Ear-tag numbers, if present;
9. (i) The method of hunting used, such as bait, hound, still hunting, or stalking;
10. (j) The number of days spent hunting for bear;
11. (k) The weapon type and caliber, if appropriate;
12. (l) Whether a guide was employed and, if yes, the guide’s name and guide tag number;
13. (m) If the bear was killed over hounds, the name of the owner(s) of the hounds; and
14. (n) The signature of the hunter subject to the penalties for making unsworn false statements under RSA 641:3.
