In response to any notice of missing information, or to any request for additional information under this part, the applicant shall:

1. (a) Specifically identify how each request or comment has been addressed;
2. (b) If revised plans and specifications are being submitted, call attention to the changes on the revised plans and specifications and add a revision date to each page that has been changed;
3. (c) Sign the submittal in accordance with Fis 1004.05; and
4. (d) Send a copy of the response, with a cover letter stating the reason(s) for providing the additional information, to the referring state agency for inclusion in the record of the permit proceeding.
