Within 30 days of the completion of each training given, the director shall file a report with the board showing:
- (a) The name and address of the training program;
- (b) The name of the training;
- (c) Which component(s) of those listed in Fam 604.02 was addressed by the training;
- (d) The beginning and ending dates of the training;
- (e) The total number of hours of the training;
- (f) The names of the trainers;
- (g) The names of the specialists, if any; and
- (h) The names of the attending students.
