A certified training program shall advise the board promptly of changes in the following information:

1. (a) The training program’s full name;
2. (b) The training program’s mailing address(es);
3. (c) The training program’s business telephone number(s) and fax number(s);
4. (d) The hours at which the director of the training program can be reached;
5. (e) The training program's e-mail address, if the program has one;
6. (f) The name of the authorized signer;
7. (g) The names and qualifications of the director, trainers, and specialists; and
8. (h) Updated information or revised documents whenever there is a change in the category of information or in the documents submitted at the time of application for certification.
