The 90-day deadline in Fam 302.01(e) shall be waived by the board for up to 52 weeks from the date the initial application was received by the board, on the basis of a letter identifying at least one of the following reasons for the inability to comply with the deadline:

1. (a) Personal illness sufficiently serious to prevent the person requesting the waiver from engaging in his or her usual work for at least 2 consecutive weeks;
2. (b) Active military duty;
3. (c) Death in the household or of a member of the family;
4. (d) Illness in the household or of a member of the family which is sufficiently serious to prevent the person requesting the waiver from engaging in his or her usual work for at least 2 consecutive weeks; or
5. (e) The destruction of records by fire, flood, or other accidental or natural disaster.
