1. (a) The applicant for an initial or renewal certificate shall sign and date the CCM Application.

2. (b) The applicant’s signature shall constitute certification that:

   1. (1) The applicant is aware that:

      1. a. The certification applies only for culverts 48 inches in diameter and smaller;
      2. b. The certification does not apply to culverts that have a history of being overtopped;
      3. c. The certification does not apply to culverts within 100 feet of a prime wetland or within ¼ mile of a designated river; and
      4. d. The Routine Roadway BMPs can be found at: https://www.nh.gov/dot/org/projectdevelopment/environment/units/program-management/documents/RR_V.9_FINAL_3-14-19.pdf;

   2. (2) The applicant will:

      1. a. Follow the Routine Roadway BMPs; and
      2. b. Submit the required quarterly reports to the department;
   3. (3) The applicant has successfully completed an approved training program and exam or is a professional engineer licensed in the state of New Hampshire in good standing and with appropriate knowledge to oversee culvert maintenance projects, or both;
   4. (4) The information provided on or with the application form, as applicable, is true, complete, and not misleading to the best of the applicant’s knowledge and belief; and

   5. (5) The applicant understands that:

      1. a. The submission of false, incomplete, or misleading information is grounds for denying the application or revoking any certificate that is issued based on the information; and
      2. b. He or she is subject to the penalties for falsification in official matters, currently in RSA 641.
