The baseline documentation report required by Env-Wt 803.01(h)(4) or Env-Wt 808.09(c)(5) shall:

1. (a) Describe the condition of the property(ies) that will be subject to the conservation interest as of the time of the transfer or acquisition of conservation restrictions;
2. (b) Identify the property(ies) on a tax map and USGS map;
3. (c) Include color photographs of all areas that provide aquatic resource functions and values; and
4. (d) Be signed by the conservation interest holder and the property owner.
