The applicant shall submit the following with the completed application form required in Env-Wt 311.04:

1. (a) A copy of a town tax map showing the subject property, the location of the project on the property, and the location of properties of abutters with each lot labeled with the name and mailing address of the abutter;

2. (b) Dated and labeled color photographs that:

   1. (1) Clearly depict:

      1. a. All jurisdictional areas, including but not limited to portions of wetland, shoreline, or surface water where impacts have or are proposed to occur; and
      2. b. All existing shoreline structures; and
   2. (2) Are mounted or printed no more than 2 per sheet on 8.5 x 11 inch sheets;
3. (c) A copy of the appropriate USGS map or updated data based on LiDAR at a scale of one inch equals 2,000 feet showing the location of the subject property and proposed project;
4. (d) A narrative that describes the work sequence, including pre-construction through post-construction, and the relative timing and progression of all work;
5. (e) For all projects in the protected tidal zone, a copy of the recorded deed with book and page numbers for the property;

6. (f) If the applicant is not the owner in fee of the subject property, documentation of the applicant’s legal interest in the subject property, provided that for utility projects in a utility corridor, such documentation may comprise a list that:

   1. (1) Identifies the county registry of deeds and book and page numbers of all of the easements or other recorded instruments that provide the necessary legal interest; and
   2. (2) Has been certified as complete and accurate by a knowledgeable representative of the applicant;
7. (g) The DataCheck Tool results containing the identification number from the department as well as any ecological review requests made to the department pursuant to Env-C 800, communications and information related to the requested ecological review, and the results of the ecological review pursuant to Env-C 800;
8. (h) A statement of whether the applicant has received comments from the local conservation commission and, if so, how the applicant has addressed the comments;
9. (i) For projects in LAC jurisdiction, a statement of whether the applicant has received comments from the LAC and, if so, how the applicant has addressed the comments; and
10. (j) If the applicant is also seeking to be covered by the state general permits, a statement of whether comments have been received from any federal agency and, if so, how the applicant has addressed the comments.
