1. (a) To register routine roadway maintenance activities, the responsible party shall complete and submit to the department the “Registration for Routine Roadway Maintenance Activities” form as listed in (b), below, that has been signed and certified as specified in Env-Wt 311.11.

2. (b) The registrations required by (a), above, shall be as specified in Table 309-1, below:
