Appendix D shall include, at a minimum, the telephone numbers and email addresses of all of the following individuals, entities, and local and state agencies holding official copies of the EAP:

1. (a) The local emergency responders, including police and fire departments;
2. (b) The regional and local dispatcher(s) for local emergency responders;
3. (c) The local emergency management director(s);
4. (d) The New Hampshire department of safety, division of emergency services and communications (911);
5. (e) The New Hampshire department of environmental services, division of water, dam bureau;
6. (f) The New Hampshire department of safety, division of state police;
7. (g) The New Hampshire department of transportation, division of operations, transportation systems, management & operations;
8. (h) The appropriate district office(s) of the New Hampshire department of transportation, division of operations, bureau of highway maintenance;
9. (i) The New Hampshire department of safety, division of homeland security and emergency management; and
10. (j) Any other person or entity included on the notification flowchart included in the EAP.
