Format of the EAP

- (a) The EAP shall be submitted on 8½ x 11 paper, hard copy, and in pdf format.
- (b) In order to facilitate the development of an EAP, the owner may use the EAP template found at the following link: https://www.des.nh.gov/climate-and-sustainability/storms-and-emergencies/dam-emergency-action-plans.
(c) The content of the EAP shall be standardized as follows:
- (1) Table of Contents;
- (2) Section 1, Notification Flowchart;
- (3) Section 2, General Responsibilities;
- (4) Section 3, Preventive Action; and
(5) Appendices, including:
- a. Appendix A, Project Description;
- b. Appendix B, Impact of Breach;
- c. Appendix C, Inundation Map; and
- d. Appendix D, Posting of the Plan.
- (d) The required contents of each section or appendix shall include the information identified in Env-Wr 504.03 through Env-Wr 504.09, as applicable.
