If the dam will be a low hazard, significant hazard, or high hazard structure, the owner shall submit plans and specifications which shall, at a minimum, include the following:
- (a) Scaled plan view;
- (b) Scaled cross-sectional view of dam through the outlet, showing elevations of inlet and outlet, watertight connections, embankment material;
- (c) Design of and cover treatment for erosion-resistant embankment slopes;
- (d) Gradation analysis for all structural soils and fill materials to be used during construction;
- (e) Compaction specifications in accordance with Env-Wr 403.02(d) and (e);
- (f) Construction sequence;
- (g) Sufficient detail to show the location of the proposed work in relation to the entire dam; and
- (h) A written OMRF as described in Env-Wr 303.04.
