- (a) An "Application for Permit to Construct or Reconstruct a Dam" form, NHDES-W-02-001, May 2024, available at https://onlineforms.nh.gov/?formtag=NHDES-W-02-001, shall be filed by the owner(s) of the dam, and the owner(s) of the property, if different than the dam owner, on which the dam is proposed to be located.
- (b) An application relative to emergency measures pursuant to Env-Wr 303.08 and Env-Wr 303.09, shall be filed by the owner(s) of the dam.
