1. (a) The owner shall provide the following information to any prospective owner prior to transferring:

   1. (1) Any and all deeds, easements, agreements, or other rights associated with the dam;
   2. (2) The state dam inventory number assigned by the department;
   3. (3) Any letters or orders issued by the department relative to the condition of the dam; and
   4. (4) The existing OMRF.

2. (b) Within 30 days of the transfer of ownership of a dam or the property on which a dam is located, the new owner shall inform the department in writing of the transfer of ownership by providing the following:

   1. (1) The name of the former owner;
   2. (2) The state dam inventory number assigned by the department;
   3. (3) The name, mailing address, email address, and daytime telephone number of the new owner;
   4. (4) The tax map and lot number of the dam;
   5. (5) The volume and page number of the deed which transferred ownership of the dam or the property on which the dam is located;
   6. (6) An updated OMRF in accordance with Env-Wr 303.04, if the dam is classified as low, significant, or high hazard; and
   7. (7) An updated EAP, if one is required for the dam, in accordance with Env-Wr 500.
