1. (a) Each sludge hauler permit holder shall record the following information for each load of sludge:

   1. (1) The date the sludge is transported from the generator’s site or facility;
   2. (2) The generator’s name, address, and daytime telephone number;
   3. (3) The quantity of sludge, in wet tons;
   4. (4) The type of sludge, such as class B biosolids or sludge that has not received sludge quality certification;
   5. (5) The name, address, and telephone number of the hauler permit holder and of the driver of the motorized vehicle if other than the permit holder;
   6. (6) The name, address, and telephone number of each site, facility, solid waste facility, or wastewater treatment plant to which the sludge or class B biosolids is delivered; and
   7. (7) The date the sludge was delivered to the site or facility.
2. (b) The information required by (a), above, shall be maintained in the vehicle used to transport the sludge while the material is being transported.

3. (c) The hauler shall:

   1. (1) Retain the records for a minimum of 5 years after expiration of the hauler permit to which the records relate; and
   2. (2) Make the records available for review by the department during normal business hours.
