The applicant shall provide the following financial information:

1. (a) The amount of financial assistance and loan term requested;
2. (b) The most current estimate of total project cost, itemized as to major facilities and related expenses such as land and easement costs, fees for engineering or other consulting services, fees for financial services, and contingencies;
3. (c) For construction projects, the percentage of total estimated costs allocated to water efficiency, energy efficiency, green projects, or other environmentally-innovative design;
4. (d) If the total estimated cost of the project exceeds the requested loan amount, the amount and anticipated source of each additional source of funds for the project;
5. (e) A description of each source of loan repayment funds;
6. (f) The applicant’s Unique Entity Identifier (UEI);
7. (g) For an applicant that is not a governmental entity, in the most recent completed fiscal year of the organization to which the UEI belongs, a statement of whether the organization received the threshold amount or more in U.S. federal contracts, subcontracts, loans, grants, subgrants, or cooperative agreements, or any combination thereof as required for prime recipient executive compensation reporting in accordance with Public Law 109- 282, the Federal Funding Accountability and Transparency Act of 2006 as amended; and
8. (h) An estimated schedule of disbursements.
