1. (a) Within 90 days from the receipt of a complete application for a groundwater discharge permit, the department shall issue a written decision to issue a permit or deny the application.
2. (b) If the decision is to issue a permit, the permit shall be valid for 5 years from the date of issuance, subject to renewal.
3. (c) The department shall grant the permit if it determines that the application demonstrates compliance with Env-Wq 402.10, Env-Wq 402.11, Env-Wq 402.23, and Env-Wq 402.24.
4. (d) If the department denies the application, the written decision issued pursuant to (a), above, shall state the reason(s) for the denial.
