1. (a) An applicant for certification, certification renewal, reciprocal certification, or retesting shall sign and date the applicable application form.

2. (b) The applicant’s signature shall constitute certification that:

   1. (1) The information on the application form is true, complete, and not misleading to the best of the applicant’s knowledge and belief;
   2. (2) If certification is granted, the applicant agrees to comply with Env-Wq 304, including the wastewater code of ethics in Env-Wq 304.25; and
   3. (3) By submitting the online form, I certify that I am the applicant listed in this application.
