In addition to verifying the information described in Env-Wq 2102.29(c), the agriculture water user shall provide the information identified below to the department:

1. (a) The monthly volume of withdrawal and discharge for each registered source and destination, respectively;
2. (b) The monthly volume(s) of water transferred to or from another facility;
3. (c) The units of measurement;
4. (d) The method of measurement;
5. (e) If water usage is determined based on a unit water use rate, the water user’s process for determining the unit water use rate; and

6. (f) Any changes in registration information or water use that have occurred since the previous report, including but not limited to:

   1. (1) The addition of a new source or relocation or abandonment of an existing source;
   2. (2) The addition of a new destination or relocation or abandonment of an existing destination;
   3. (3) A change in method of measurement; and
   4. (4) An explanation of the conditions that resulted in the changes under (1) through (3), above.
