To apply for EQ certification, the applicant shall provide the following information:

1. (a) Complete and submit the "Exceptional Quality Septage Certification Application" form, NHDES-W-09-026, February 2025, available at: https://onlineforms.nh.gov/?formtag=NHDES-W-09-026;
2. (b) A description of the process to achieve EQ pathogen and vector attraction reduction requirements, including the applicable 40 CFR part 503 citation; and

3. (c) A solids quality report including the following:

   1. (1) A description of the treatment facility and solids and filtrate treatment process;
   2. (2) A chronological summary of analytical data from the previous 3 years, if available, for the required parameters, presented in tabular format; and

   3. (3) The results of testing required in Env-Wq 1610.05(d), including:

      1. a. Laboratory reports; and
      2. b. A tabular summary of the results.
