1. (a) Each year that a septage holding tank(s) permit is valid, the holder shall submit an annual report to the department by the last business day of January for the prior calendar year, regardless of whether or not septage was received or transferred within the prior calendar year.

2. (b) The annual report shall contain the following information:

   1. (1) The location of the septage holding tank(s), including street address and town;
   2. (2) The septage holding tank(s) permit number;
   3. (3) The owner’s name, if other than the permit holder;
   4. (4) The total volume of septage, in gallons, received by month and municipality of origin; and

   5. (5) Information for the final disposition of the septage to include:

      1. a. The facility or site name;
      2. b. The amount transferred in gallons;
      3. c. The permit number of the facility or site; and
      4. d. The receiving state of the facility or site.
