Persons filing for a shoreland permit by notification under RSA 483-B:5-b, I shall submit the following to the department:

1. (a) A complete notification form as described in Env-Wq 1406.17 that has been signed by the owner of the property as specified in RSA 483-B:5-b, I(a) and Env-Wq 1406.08;
2. (b) The plans required by RSA 483-B:5-b, I(a) that clearly and accurately depict the work to be completed relative to the reference line of the jurisdictional waterbody;
3. (c) The photographs required by RSA 483-B:5-b, I(a) of the area to be impacted;
4. (d) Identification as required by RSA 483-B:5-b, I(a) of the project criteria that qualify the project for a permit by notification; and
5. (e) The fee required by RSA 483-B:5-b, I(b), unless exempted by RSA 483-B:5-b, III.
