1. (a) The applicant for a shoreland permit shall submit to the department postal receipts, or copies of the receipts, to verify that the notices required by RSA 483-B:5-b, IV-a, reprinted in Appendix C, have been delivered.

2. (b) The written notices required by RSA 483-B:5-b, IV-a, shall be sent at the time of submittal of the application to the department and shall include:

   1. (1) The street address and municipality of the property that is the subject of the application;
   2. (2) A clear statement that an application has been filed under RSA 483-B;
   3. (3) The date that the application was or will be filed with the department; and
   4. (4) For a project that requires notice to the appropriate local river advisory committee because a portion of the project is located within ¼-mile of a river or river segment designated under RSA 483, a copy of the completed and signed application.
