1. (a) The grant recipient shall submit a written report to the department at the completion of the project.

2. (b) The report required by (a), above, shall include:

   1. (1) A description of any deviations or modifications to the project as described in the application were made, and if so, why;
   2. (2) Whether the goals of the project were achieved, and if not achieved, the likely reason(s) why;
   3. (3) A table summarizing and enumerating the tasks and deliverables realized/achieved for each task outlined in the project proposal.
   4. (4) The final actual budget for the project, highlighting any amendments made during the course of the project, and including the types and amount of match contributed towards the grant.
