1. (a) The user shall provide notice to the WRBP in accordance with this section in the event of any of the following:

   1. (1) A discharge violation detected through analysis;
   2. (2) Unanticipated bypass or slug discharge; or
   3. (3) Upset that exceeds applicable pretreatment standards.

2. (b) When required to notify the WRBP, the user shall provide the following:

   1. (1) Oral notice by telephone within 24 hours of the time the user becomes aware of the event; and
   2. (2) A written report within 5 business days of the time the user becomes aware of the noncompliance.

3. (c) The written report required by (b)(2), above, shall include:

   1. (1) A description of the event and its cause;
   2. (2) The duration of the event, including exact dates and times;
   3. (3) If the event has not been corrected, the anticipated time it is expected to continue; and
   4. (4) Steps taken or planned to reduce, eliminate, and prevent recurrence of the event.
