The applicant also shall provide the following information regarding pool or spa operations and safety:

1. (a) Information on emergency equipment, including whether there are or will be:

   1. (1) For pools:

      1. a. A 12-foot pole with body hook as required by Env-Wq 1107.06(a)(1);
      2. b. A type IV rescue flotation device as required by Env-Wq 1107.06(a)(2); and
      3. c. Whether there is or will be a rope with floats at the break point in a pool as required by Env-Wq 1108.18(f), if applicable;
   2. (2) A first aid kit as required by Env-Wq 1107.06(a)(3);
   3. (3) Depth markers on the deck and vertical walls as required by Env-Wq 1108.18; and
   4. (4) For spas, a functioning clock that is visible to each spa;

2. (b) Information on signage, including:

   1. (1) Whether there are or will be “No Lifeguard” signs at all points of access as required by Env-Wq 1107.02(c), if there will be no lifeguard or other individual to enforce safety standards;
   2. (2) Whether patron safety rules are or will be posted as required by Env-Wq 1107.04(a)(2) or Env-Wq 1107.04(e)(2), as applicable;
   3. (3) Whether there is a phone or a sign identifying the nearest location of a phone as required by Env-Wq 1107.04;
3. (c) For outdoor PBFs, confirmation that the security fencing and self-closing, self-latching gates meet the requirements specified in Env-Wq 1108.08(a);
4. (d) The anticipated maximum bather load;
5. (e) Information on the availability of toilet and shower facilities as specified in Env-Wq 1108.04 and Env-Wq 1108.01, respectively;

6. (f) Information on the water source for the PBF, including:

   1. (1) The source of water, and whether it is a public water supply, a private groundwater supply, or surface water;
   2. (2) If the source water is a public water supply, the name and municipality of the water supply; and
   3. (3) Whether water service to the PBF meets the requirements of Env-Wq 1108.10;
7. (g) Whether sewage and other wastewater, exclusive of filter backwash and pool drainage, will be discharged into a public sewer or a subsurface disposal system approved under RSA 485-A:29-44 and Env-Wq 1000;
8. (h) Whether the filter will be backwashed;
9. (i) Confirmation that the discharge or disposal of filter backwash and pool drainage meets the requirements of Env-Wq 1109.07(g); and
10. (j) Information on the air-handling system to show compliance with Env-Wq 1108.21.
