1. (a) To apply for a general ITA or general ITA with conditions, the ITA applicant shall submit the following in writing to the department:

   1. (1) All of the information specified in Env-Wq 1024.05 for a provisional ITA application;
   2. (2) A clear and concise summary of the results of the performance testing conducted pursuant to the provisional ITA; and
   3. (3) If the application is for a general ITA with conditions, a list of the conditions the ITA applicant is asking to have in the approval, which may include a manufacturer’s design manual.
2. (b) If information addressing more than one category of Env-Wq 1024.05(a)(3) through (13) is found within the same document, the ITA applicant shall identify on a separate sheet of paper which categories are covered in that document, listed by page number.
