All facilities operating under a permit-by-notification or seeking a permit-by-notification in accordance with this part shall submit the following documentation to the department no later than 30 days after receipt of a request from the department for such documentation:

1. (a) The written operating plan required by Env-Sw 1202.02(a)(2)a.;
2. (b) The written closure plan required by Env-Sw 1202.02(a)(3)a.;
3. (c) Any operator certifications required by Env-Sw 1202.05(d);
4. (d) Proof of compliance with the siting criteria set forth in Env-Sw 1203;
5. (e) Proof of notification in accordance with Env-Sw 311.04(b) and (c);
6. (f) Proof that adequate funds in an amount no less than the amount calculated pursuant to Env-Sw 1403.02, are readily available to cover the cost of facility closure; and
7. (g) Any additional documentation required by Env-Sw 407, Env-Sw 507, and Env-Sw 607, as applicable.
